Important Notice: Discontinuation of Delta Dental PPO Plan

The MA/MOSES Health and Welfare Trust will be discontinuing the Delta Dental PPO dental insurance plan effective December 31, 2025.
What This Means for You:
You will need to choose a new dental coverage option before December 12, 2025. You may either:
- Enroll in the Altus dental plan, or
- Switch to the Open Reimbursement Plan offered by the MA/MOSES Health & Welfare Trust
In the coming weeks, you will receive a package in the mail containing the new Decision Guide, which provides important information about the Open and Altus plans. Included with this letter is the 2026 Altus Benefit Summary for your review.
If you would like to make your choice now, please use the Enrollment Form on the back of this letter to indicate your choice. I strongly encourage you to make your selection before the December 12th deadline to ensure timely processing of any necessary payroll deduction changes.
